HCG is a protein hormone produced by the placental chorionic trophoblast cells with gonadotropin development. Its primary function is to stimulate the transformation of the corpus luteum from the menstrual corpus luteum to the gestational corpus luteum, while promoting the continuous secretion of estrogen and progesterone to maintain endometrial morphology, fostering the development of uterine decidua, and maturing the placenta—critical for maintaining pregnancy. HCG plays an essential role in pregnancy diagnosis, trophoblast management, and is pivotal in standardizing pregnancy tests and trophoblast screenings.

 

 

 

HCG is composed of two peptide chains, α and β. The α chain is analogous to the α chains of pituitary-secreted hormones: FSH (Follicle Stimulating Hormone), LH (Luteinizing Hormone), and TSH (Thyroid Stimulating Hormone). As such, cross-reactivity among these hormones can occur. The β chain is unique to HCG. In clinical diagnostics, specific anti-β-HCG antibodies are used to avoid cross-reactivity with other hormones during early pregnancy diagnostics.

Clinical Importance of HCG

The clinical value of HCG extends beyond early pregnancy detection to include applications in ectopic pregnancy, prenatal screening for congenital anomalies, and tumor screening.

 

 

Early Pregnancy Test

Distinguishing Normal from Abnormal Pregnancies: For instance, ectopic pregnancies often suffer from inadequate blood supply to the implantation site, leading to underdeveloped trophoblast cells, reduced HCG secretion, and subnormal hormone levels.

 

Diagnosing Miscarriage: In incomplete miscarriage, residual placental tissue in the uterus results in a qualitatively positive HCG reading. However, in complete miscarriage or stillbirth, HCG can be negative.

 

Tumor Marker: Determination of Free β-hCG is clinically beneficial in diagnosing gestational trophoblastic diseases (hydatidiform mole, choriocarcinoma) and some testicular tumors. Free β-hCG typically accounts for a large proportion of total HCG in these conditions, and some tumor cells may only secrete Free β-hCG. For men, elevated HCG levels are a significant indicator for screening testicular cancer and yolk sac tumors.

 

Screening for Trisomy 18/21: Free-β-HCG is a required indicator for screening for congenital dementia and neural tube defects.
